Quality Improvement Plan Questions and Answers

What is a Quality Improvement Plan (QIP)?

A QIP is a formal, documented set of organizational priorities focused on quality and quality improvement objectives. Using priority indicators, we have made a commitment to patients, staff, health care partners and our communities to improve quality through a series of concentrated targets and actions. QIPs are used to assist organizations in delivering quality programs and services.


What is the objective of a QIP?

A QIP is owned by each Local Health Integration Network (LHIN) and is a way for that organization to focus its efforts on key quality improvement priorities. A QIP helps to achieve the following objectives:

  • We may be undertaking a range of quality improvement projects at any given time. A formalized QIP allows us to organize and prioritize these initiatives. These projects are consistently aligned with the goals of each LHIN and also contribute to greater health care system objectives.
  • Supports the goals and principles of the Excellent Care for All Act (ECFAA), including accountability, transparency, and high-quality patient care.
  • QIPs also allow for patients, community members, providers and health care partners to be engaged in the quality of care we provide.
  • QIPs are an opportunity for the LHINs to also promote their key quality initiatives as they align with greater health-system objectives. We are active participants in and supporters of shared health system improvement, putting patients first.
  • QIPs allow us to account for its commitments to continuous quality of care improvement.


Who develops a QIP?

The QIP process is one of engagement and takes into account the needs of patients and of our unique and diverse communities. The Board of Directors, senior management, and various stakeholders are all involved in a QIP development.


What is a priority indicator?

A priority indicator is a measurable target that has been identified within the QIP. Priority indicators were selected based on demonstrated alignment to shared, cross-sectoral efforts to improve safer transitions of care, patient experience, and improved access for Ontarians. Achieving system-wide change in these areas requires every sector and every organization to prioritize quality improvement.


How are QIPs monitored and reported?

We submit our QIPs to Health Quality Ontario (HQO) annually. In addition to developing a QIP, each CCAC is tasked to ensure continuous monitoring and tracking of the performance of the priorities and commitments set out in our QIPs.


What is the QIP Narrative?

The QIP Narrative provides context to each CCAC's QIP work plan and allows us to define what its quality improvement priorities are for the year. The QIP narrative defines our vision for quality improvement.


What is the QIP Work Plan?

The QIP work plan is a designated set of quality objectives, defined by priority indicators and measurable targets that are monitored and tracked throughout the year.
