Quality Improvement Plans

Our goal is to deliver high-quality, integrated care to our patients, as is reflected in our vision – Outstanding care, every person, every day. Additionally, we are key collaborators in the health care system, supporting system integration with our health care partners in order to achieve the objective of delivering the best possible care to Ontarians.

 

In keeping with this commitment, we have developed Quality Improvement Plans (QIP) aligned with the key principles found in the Excellent Care for All Act (ECFAA) and as directed by the Ministry of Health and Long-Term Care. The QIP is one tool that helps to define organizational priorities focused on delivering patients the best care possible through quality initiatives and continuous quality improvement.  Under this process, the QIP is also central to progressive health system transformation by ensuring all health sectors partner for quality, patient-centered care.

 

What is a QUALITY IMPROVEMENT PLAN (QIP)?

A QIP is a formal, documented set of organizational priorities focused on quality and quality improvement objectives that are reviewed and posted annually. Using priority indicators that have been identified by The Ministry of Health and Long-Term Care, we have made a commitment to patients, staff, health care partners and our communities to improve quality through a series of concentrated targets and actions.

We will report on indicators, which in collaboration with other health care partners can make a significant positive impact to patients and the health care system. These core set of indicators support provincial priorities with targets in the areas of safety, effectiveness, access and focuses on patient-centered care.

North Simcoe Muskoka QIP

We are committed to the improvement of both the health experience and health outcomes of our patients and values safety and quality and through continuous quality improvement and a culture of excellence.

In leveraging learnings from the 2017‐18 QIP, home and community care aims to achieve targets in:

  • ​Improving access to care while supporting a greater number of people with high care needs through the achievement of our wait time targets.
  • Improving patient safety in the home while enhancing the corporate safety culture.
  • Improving staff safety by continuing workplace violence prevention work.
  • Improving effectiveness through reduced unplanned and avoidable emergency department visits and hospital admissions through collaboration with health system partners.
  • Improving patient‐centeredness by efficiently delivering better patient and caregiver experiences, as well as improved health outcomes.

Our focus in 2018-19​ is on a total of eight indicators with the aim to drive improvements in experience and care outcomes, integration, value, and safe care coordination and care transitions.  Of those indicators, six are aligned to the home care sector and two are unique to North Simcoe Muskoka.

 

At a Glance​

Home Care Sector Indicators:

  1. Improve overall patient experience with care.
  2. Reduction in unplanned emergency department visits experienced by home care patients that occur within 30 days of their transition home from hospital.
  3. Reduction in unplanned hospital readmissions experienced by home care patients that occur within 30 days of their transition home from hospital.
  4. Reduction in wait times, for all patients requiring nursing services and for patients with complex needs who require personal support services, targeted to align to the provincial 5-day wait time measure.
  5. Wound Care, specifically improvement in the provision of care for people who have developed a diabetic foot ulcer.​
  6. Reduction in time to acknowledge complaints. ​

Local Indicators:

  1. Improve patient and caregiver experience regarding how we and our contracted service provider staff address pati​ent safety.
  2. Improve overall patient and caregiver experience with end of life/palliative care​.

 

Our QIP initiatives are aligned with our strategic priorities. Of strategic importance is being patient-centered and empowering the patient as an active partner in their care and wellbeing; implementing evidence-based practice to drive quality and safety and building and leveraging strategic partnerships with our system partners at the local and provincial level to improve patient care.      

 

Quality Improvement Achievements from the past year

Access to timely care is a measure of the quality of health care that a person received. Five-day wait times for nursing and complex personal support were identified as long-term QIP priorities within the Home and Community Care sector. We identified 5 day wait time for nursing and complex personal support as a priority area in the 2017-2018 QIP. Significant improvements were made for both complex personal support services and nursing wait times, any by November 2017, the targets were exceeded for nursing with 96% of patients being seen within 5 days or less.

Patient safety continued to be a priority area for the North Simcoe Muskoka LHIN and efforts in 2017-18 were focused on reducing medication errors, and on establishing proactive approaches to quality improvement specific to patient safety, both within our organization and in collaboration with other health system partners.

The North Simcoe Muskoka LHIN is proud to champion a just culture of patient safety and quality, and will continue to partner with patients, caregivers and other health care providers in advancing the health care quality agenda across our local health system. ​

1) A narrative document that provides a brief overview of our QIP, highlighting some of our key priorities for the 2018-2019 fiscal year.
