Home and Community Care Support Services North Simcoe Muskoka is one of 14 Home and Community Care Support Services organizations in Ontario with a focused mandate to deliver local health care services such as home and community care, access to community services and long-term care home placement.

Previously, we were known as the North Simcoe Muskoka Local Health Integration Network (NSM LHIN), at a time when we also led local health care system planning and funding. These planning and funding functions are now part of Ontario Health, a government agency responsible for ensuring Ontarians continue to receive high-quality health care services where and when they need them. 

Prior to 2017, home care services were funded by the NSM LHIN, and operated as the NSM Community Care Access Centre (CCAC).
