Anyone can call us to inquire about home and community care services. We have a receptive team of employees who can answer your questions and help you access the care you need.

HOW CAN WE HELP YOU?

  • We work with people to determine the goals of their care.
  • We help people make the transition from hospital to home.
  • We support people to stay in their homes and communities.
  • We give people access to supportive care settings.

If you would like to access care, or if you are currently receiving care and have questions, call us at:

1-888-721-2222 or 705-721-8010

8:30 a.m. to 8:30 p.m.

There are a range of resources to support you with home and community care, such as the Guide to our Services and the Caregiver Support Guide.

On May 3, 2017, all home and community care staff at the North Simcoe Muskoka Community Care Access Centre (CCAC) were transferred to the North Simcoe Muskoka Local Health Integration Network (LHIN). This transfer is part of the efforts of patients first: The Province of Ontario's Action Plan for Health Care, which aims to increase access to care, reduce wait times and improve the patient experience.

There will be no interruption in your care. You will have the same home care providers. Office locations will remain the same. You can call the same phone numbers, write to the same email addresses, and visit the same websites to get in touch with us.

As always, our priority is timely access to safe and quality health care for patients, caregivers and families.
