Health Partner Gateway

​​H​ome and community care services and staff transferred from Community Care Access Centres (CCACs) to Local Health Integration Networks (LHINs). Home and community care services are provided through Ontario’s 14 LHINs. LHINs plan, integrate and fund local health care and ​deliver and coordinate home and community care as well.

Health Partner Gateway

​The Health Partner Gateway (HPG) provides a single electronic solution to securely exchange health information between a LHIN and its partners. It has been built to interoperate with longer term e-health initiatives and to enable partner access to health information held within LHIN tools and applications such as the Client Health and Related Information System (CHRIS).


The Health Partner Gateway (HPG) makes it simple to share information between LHIN and their health p​artners, in a safe and secure way.

​​HPG Health Partner Reference Guides

The Reference Guide for Health Partners has been separated into individual modules to address functionality within each of the different areas of HPG.  Module 1 is intended for all users as it covers general concepts and functionality that are common to all external HPG users.  All other modules are more specific and intended for users that have been assigned the corresponding role/roles.

Multiple User Account Set-Up Form - used when an organization is initally registering users in HPG.

User Maintenance Form - used when either adding or removing individual users from HPG or an update needs to be made to an existing user's account (e.g. name change, change in role assignments, etc.)

External Communication Reports Reference Card -  This reference card outlines the new layout to the Service Referral and Frequency Change Report introduced in CHRIS 2.5/HPG 3.3, as well as enhancements to the report functionality in CHRIS 2.5.2. This document is a companion document to the HPG Overview of External Communications Webinar held on Monday, December 8 2014.​
