On May 1, 2017, home and community care services and staff begin to transfer from Community Care Access Centres (CCACs) to Local Health Integration Networks (LHINs). After transfers are completed by June 21, 2017, home and community care services will be provided through Ontario’s 14 LHINs. LHINs plan, integrate and fund local health care and will deliver and coordinate home and community care as well.
