Care in Community

Living at home is often a high priority for patients and their families. While in many cases this can be accomplished successfully with the right health-care supports in place, people may also find other assistive community services helpful.

These services can contribute to enhanced independence, enjoyment and relaxation as well as much-needed caregiver support.

For example, community services may include:

  • Meal delivery and dining programs
  • Homemaking and home help
  • Transportation services
  • Community dining
  • Friendly visits

People are encouraged to consider the benefits of other services available in the community. These services can help reduce the stresses of daily living for both the individual and the caregiver.

Our community clinics provide services that address specific health-care needs, such as IV therapy, wound care, or rehabilitation therapy. Visits are arranged by your Care Coordinator and are by appointment only. Community clinics are staffed by specially trained nurses and are not the same as visiting a walk-in medical clinic
