What is the Home First Approach?
Home First is an approach to managing acute care patients differently, identifying patients at risk of ALC LTC to reduce designation.
The focus of Home First is on making discharge home possible and avoiding discussion about LTC placement in hospital with earlier involvement of CCAC (before ALC designation/LTC referral)
Home First mobilizes community resources—CCAC and Community Support Services (CSS) to enable discharge home.
Avoiding deterioration in hospital or judging patients as ‘unable to return home’ making conversion from acute care to ALC/LTC in hospital a last resort.
