Long-term Care Homes

There are 36 Long-Term Care Homes throughout Southeastern Ontario. Some homes are privately owned and some are non-profit homes operated by municipalities or charitable organizations. All homes are licensed and inspected by the Ontario Ministry of Health. The standards of care are governed by provincial legislation and each home is accountable for the quality of care, programs and services it provides.

Placement Priority Categories for Individuals

1. Crisis - People requiring immediate admission resulting from the person's condition or circumstances.

2. Spousal/Partner Reunification - A spouse/ partner seeking to be reunited with their loved one who already resides in that specific LTC home. The applicant must meet eligibility criteria in his or her own right.

3. Ethno-cultural/Religious Groups - Person of the religion, ethnic origin or linguistic origin primarily served by the home who meets one of the conditions listed for 4A.

4. A. Person living in the community who has high care needs, requiring personal care at frequent intervals throughout the day, but can be supported at home until a bed becomes available. Also includes a person occupying a bed in a hospital waiting for a long-term care bed, and anyone residing in an LTC home wishing to transfer to their first choice home.
B. Person living in the community who does not meet the care requirements to be a 4A, or someone already residing in a LTC home and waiting to transfer to another home that is not their first choice facility.

Re-admission Category
Applicant was a long-stay resident in the home but was discharged after a medical or psychiatric absence that exceeded the time permitted in regulation.   Note: applies only to home from which the person was most recently discharged; or
Applicant was a long-stay resident in the home but was discharged due to an emergency in the home Note: applies only to home from which the person was most recently discharged.

Veteran's Priority Access Beds are designated beds that are accessible by qualifying veterans. These individuals are a higher priority for placement in these beds than non-veteran applicants.
