While CCACs serve our local geographical regions (which are aligned with Local Health Integration Networks, or LHINs and provide care to our communities in individualized ways, we are united in our efforts to ensure high quality care across the province.
We are committed to innovation and best practice standards to ensure we are meeting the needs of Ontario's population and improving the care we provide our patients.
To support these goals, we develop province-wide programs and initiatives with support from the Ontario Association of Community Care Access Centres to better enable our patients and their families to receive the support they need, when they need it.
Information & Referral
To provide the best possible care, the CCACs must have accurate, timely and secure systems that allow professionals to share resources. Our Information and Referral initiatives allow us to provide essential information to patients, family members and other partners about available community and health services – electronically, by telephone and in person. Specialized Nursing care
CCACs are hiring more nurses to provide front-line care to our most vulnerable patients: frail seniors and adults, children with complex illnesses and people who require end-of-life care. These new nurses will provide mental health and addiction support for students, rapid response nursing care for high-needs patients transitioning home from hospital, palliative care support and remote health monitoring for people with severe chronic diseases.

Across the province, we are introducing an outcome-based approach to care. Through a team-based approach, outcome-based care uses best practices to achieve positive results for people with similar care needs, such as wound care or better mobility following hip or knee surgery.

Health Links are local health-care networks which organize care around the needs of people with high care needs, whether at home or in the community. Health Links are a vital part of CCACs' primary care integration initiatives.

Our ability to provide effective patient care depends on our ability to communicate clearly, quickly and accurately with our health-care partners, and with each other. As part of our care delivery programs, we also provide extensive online resources for our patients and their family members. CCACs use the most advanced tools and information-sharing technologies, designed specially to meet our complex communication and information needs.

Recent changes to physiotherapy delivery
CCACs are now the single-point of access for all publicly funded in-home physiotherapy services. This change to physiotherapy delivery helps to streamline care and ensure that people across the province have equitable access to care, no matter where they live.
