We are seeking Care Coordinators for our Belleville, Kingston, Brockville/Smiths Falls locations.

​Care Coordinators are responsible for client assessment, determination of eligibility, admission, service planning and authorization, implementation, monitoring, reassessment, adjustment and discharge planning of all client service programs (in-home and placement), including the provision of community resource information and referral. Care Coordinators link clients with the right information and help them achieve their short and long-term health care goals.
