Job Summary:

Nurse Practitioner  [permanent full-time (70 hours bi-weekly)]

Reporting to the Senior Manager Clinical Support & Utilization, the Nurse Practitioner (NP) is primarily responsible for providing clinical care to support Long Term Care Homes (LTCHs) across the SECCAC. The NP will liaise with Hospital and Community Care Coordinators, LTCHs, Physicians, the LTCHAM Team and hospitals in the South East to facilitate hospital discharge, and prevent ED visits. The NP will function at full scope of practice to assist in meeting patients’ primary care needs and project goals.
