The South West CCAC works closely with our hospital partners. Our Care Coordinators are embedded in hospitals across the South West to ensure a direct contact to the CCAC for patients.
Remaining in hospital after the need for acute care has passed does not contribute to patient recovery. The Home First approach to care is based on the philosophy that when a person enters hospital, everyone will work together to get that person home upon discharge.
Care Coordinators work to ensure a smooth transition for patients from hospitals to home or to long-term care. Under the innovative Home First philosophy, in place in all hospitals in the South West since March 2014, Care Coordinators work to help identify patients who are at risk of becoming ALC while in hospital, and help get them the home and community care they need to be able to leave hospital as quickly as possible.
