​Welcome to a special section designed to help you find the information you need about the South West Community Care Access Centre (CCAC).The CCAC helps patients get the care they need in their homes and communities. Our people are dedicated nurses, occupational therapists, social workers and other professionals, dedicated to helping people heal at home, stay in their homes longer, and when that is no longer possible, find alternate accommodation. At the CCAC we are committed to providing outstanding care in partnership with our patients and families, and our partners across the health system. We are constantly looking for innovative ways to improve care.

  • Innovating for Better Care is our annual community report, which outlines our performance, achievements and goals for 2012-2013.
