Palliative Care Outreach Teams
The South West LHIN has five community-based interdisciplinary Palliative Care Outreach Teams (PCOT) across the region to support end-of-life care for patients and families.
The community-based consult teams work in partnership with primary care providers to support individuals receiving end of life care. Support is available 24 hours a day, seven days per week. Our interdisciplinary care teams may include: care coordinators; palliative care physicians; nurse practitioners; home visiting nurses; a pain and symptom management consultant; local hospice and bereavement professionals; and a spiritual care consultant.
Our teams will work with you at home, in long-term care or in hospital to help manage pain and other distressing physical and emotional symptoms associated with palliative care at end-of-life.
