Enabling care through technology

CCACs deliver high quality care in the home, at school and in the community. One of the key reasons we can do this so successfully is clear and quick communication with health-care partners and providers involved in your care.health care worker checking patient's leg

We have developed leading-edge electronic tools, platforms and best practices. These initiatives provide us with the technology platform required to deliver the best possible patient care, and offer you and your family timely, accurate information to support your health and well-being.

We also participate in provincial initiatives that provide our patients with additional health-related services, including Health Care Connect.

The CCAC e-health platform provides us with access to common information and the ability to support strong connections and effective communication with our partners across the health-care system, for example hospitals, primary care and community support services. This helps us better respond to people's needs and create a smooth care experience for you and your family.

You can learn more about technology-based solutions to support patient care, information sharing, planning, decision-making and communication needs by visiting the Ontario Association of Community Care Access Centres website.

Connecting South West Ontario's (cSWO's) goal is to implement a regional ehealth program that will make an individual's health information from across the continuum of care available in a timely and secure fashion at any point of care.
