Anyone can call to find out about home and community care services. We have a responsive team of staff who are able to answer your questions and help you access the care you need.
HOW WE HELP
- We work with people to identify goals for their care.
- We help people transition from hospital to home.
- We support people to remain in their homes and communities.
- We provide people with access to supportive care settings.

There are a variety of supportive resources available on home and community care services, including a Guide to our Services, and a Support Guide for Caregivers.
All home care services and staff from the South West Community Care Access Centre (CCAC) transferred to the South West Local Health Integration Network (LHIN) on May 24th, 2017. This is part of Ontario’s Patients First: Action Plan for Health Care which is increasing access to care, reducing wait times and improving the patient experience.
There will be no interruption to your care. You will keep the same home care providers. Office locations remain the same. You can use the same phone numbers, email addresses and websites to get in touch with your providers and arrange your care.
As always, our priority is timely access to safe quality care for patients, their caregivers and families.
