Care Coordinators

​​​​Care Coordinators are the heart and soul of home and community care. They not only help you understand your different care options when you are new to us, but they also ensure that you receive, and continue to receive, the ongoing care you need, when you need it.

 

Who are Care Coordinators?

Care Coordinators are regulated health professionals such as nurses, social workers, occupational therapists, physiotherapists or speech therapists. They support our clients at home and also work with clients receiving care in hospitals, doctor's offices, communities, and schools​.

What do Care Coordinators do?

Care Coordinators are connected to every part of the healthcare system and can be a single point of contact when you need health services or more information.

Care Coordinators use their health professional's knowledge and expertise in order to understand your individual needs, and make recommendations based on your needs and goals.

We work hand in hand with you and your family to find out what care you need, and then work with you to develop a care plan that is right for you – whether it's nursing care, meal delivery, a day program, or help finding a family doctor.

 

How do I connect with a Care Coordinator?
