Quality Improvement Plans
We are committed to delivering high quality care and a superior patient experience. Quality Improvement Plans (QIPs) provide guidance and direction on quality improvement initiatives, with the overall objective of improving the quality of services delivered to patients in accordance with the
Excellent Care For All Act.
Outcome-based care
Across the province, we are introducing an outcome-based approach to care as a new way of providing home and community care. Through a team-based approach, outcome-based care uses best practices to achieve positive results for people with similar care needs, such as wound care or better mobility following hip or knee surgery.
PATIENT EXPERIENCE
Feedback from our patients is the best way for us to know if we are living up to our commitment of delivering outstanding care in the Toronto Central region. View results from our Patient and Caregiver Satisfaction Survey and the number and type of complaints we received over the past year.
