Toronto Central CCAC has recently been confirmed as the lead coordinating organization for the new West Toronto Health Link, a new opportunity to work with existing and new partners to collaborate and innovate within the local Link community.
Building Health Links is about building relationships. As lead coordinating organization, Toronto Central CCAC is committed to fostering the engagement of all community health partners in building effective relationships: with clients and families, with caregivers and service providers in the community, as well as with acute and primary care sectors. These growing relationships will encourage greater collaboration and communication, so that partners working within the West Health Link can move ahead with a shared collective purpose. In addition to wrapping care around the most vulnerable people living in the West Toronto Health Link community, the West Health Link will also address needs of its children and youth.
This collaborative leadership opportunity reflects and builds on Toronto Central CCAC’s Strategic Directions: it is a key part to providing care that is responsive to our clients’ needs and choices, doing so with the support of a tightly linked team of health care providers.
Over the next 6 months, we will be focusing on preparing a readiness assessment and business plan for the official kick-off of the West Toronto Health Link in the spring of 2014.
