CCAC Wheelchair Clinics – Opening April 1, 2016

Waterloo Wellington Community Care Access Centre CCAC) is pleased to announce the opening of two WWCCAC Wheelchair Clinics. The clinics will provide patients with convenient access to Occupational Therapy (OT) seating assessment and equipment prescription, as well as greater flexibility in scheduling visits around other daily activities.

In addition to clinic visits, the Occupational Therapist service provider may also visit patients in their home to assess the home environment for wheelchair accessibility.  

Wheelchair clinic locations:

Patient inclusion criteria for wheelchair clinic:

  • Patient is able to get out of their house and lives within 30-45 min of one of the clinics (dependent on the patient's tolerance for travel).  Care Coordinator's will work with patients to arrange appropriate transportation; and

  • Patient is 18 years of age or older; and

  • Seating and/or positioning is identified as patient's primary need; or  

  • Patient is in need of wheelchair/seating adjustment or replacement.

Patient exclusion criteria: 

  • Patient requires a mechanical lift for transfers

  • Transport wheelchairs

  • Walker assessments
