Integration of Hospice Palliative Care Consultation Services in Waterloo and Wellington

GUELPH - On October 1, 2013, Hospice of Waterloo Region (HWR) will become the health service provider organization for the Wellington Hospice Palliative Care Consultation Service.

This service has been led by the Waterloo Wellington Community Care Access Centre (WWCCAC) since its inception in 1994.  Christine Bigelow, Nurse Consultant, provides palliative expertise, pain and symptom management consultation and educational support to community health care providers.  Earlier in 2013, Hospice of Waterloo Region became the health service provider organization for the same program in Waterloo Region, Waterloo Region Hospice Palliative Care Consultation Service. 

In keeping with the Waterloo Wellington Local Health Integration Network's goals of integration to minimize duplication, and leverage relationships between existing providers, HWR will leverage the relationship between the consultants and the community Hospice Palliative Care teams within Waterloo and Wellington to successfully in support access to community providers for common education, facilitating face-to-face case conferencing, and in planning care. 

This transfer reflects Hospice of Waterloo Region's strategic focus on excellence in public and professional hospice/palliative education and does not change the service mandate of the Wellington Hospital Palliative Care Consultation Service.  The Wellington Hospice Palliative Care Consultation Service will continue to be delivered by the current nurse consultant Christine Bigelow and will be based at Hospice Wellington, providing for a seamless and continued presence in Guelph and Wellington County.
