Assignment: 141 Weber Street South, Waterloo, Regular Day Shift.
Functioning as a member of an integrated wound care team this position is accountable for providing and promoting advanced wound care.
In collaboration with internal staff and health system partners, the Advance Practice Nurse:
• Collaborates with advanced wound care specialists (e.g. physician, ET Nurses, therapists, chiropodists) and LHIN Nursing Clinic Service Provider organizations in the development, implementation and evaluation of an integrated wound care clinic.
• Provides advanced wound care to patients, with a strong emphasis on assessment, treatment, teaching, health promotion and wound prevention.
• Liaises with primary care and other members of patient's health care team.
• Ensures the establishment and adoption of wound care best practices based on RNAO, Canadian Association of Wound Care and Waterloo Wellington Integrated Wound Care Program guidelines.
• Provides knowledge transfer of leading clinical practices in wound management and prevention to improve patient outcomes and patient experience.
• Teaches, mentors and coaches members of the multi-disciplinary health care team on leading clinical best practices in wound prevention and wound care.
• Champion's clinical quality improvement initiatives and drives improvement in safe, high quality and evidence based wound care practice.
- The successful candidate will possess a degree in nursing either:
- Completion of an educational program for Nurse Practitioners, registration in the Extended Class with the Nurses of Ontario, and 3 years nursing experience providing advanced wound care.
Completion of International Inter-professional Wound Care Course, preferred or College of Nurses of Ontario as a Registered Nurse at the Masters level – Advanced education in a Wound Care specialty (Completion of International Inter-professional Wound Care Course preferred), 3 years nursing experience providing advanced wound care.
- Proven leadership capabilities and success working with an interdisciplinary team and community services, combined with an understanding of the roles of health professionals, support workers, and programs related to community health and social services are required competencies.
- Computer literacy and demonstrated ability to work independently in a multidisciplinary team setting with the ability to promote effective working relationships is essential.
- Travel throughout the Waterloo Wellington LHIN will be required.
