Speedvale Avenue, Guelph, Regular Day Shift.
Reporting to the Manager, Education & OD and in collaboration with the Professional Practice Lead in Patient Services, this position is responsible to plan, design, deliver and evaluate general training and clinical practice programs, and provide learning solutions that drive performance excellence to support Best Practice at the Waterloo Wellington CCAC.
This position requires a strong commitment to working in partnership with team members and in consultation with other portfolios to ensure CCAC education achieves the goals, expectations and deliverables of a variety of stakeholders, including front line support, professional staff and management. Included is the provision of assistance to individual Care Coordinators to enhance their professional practice skills through education, practice validation activities and related mentoring, coaching and preceptoring.
The successful applicant will be required to maintain current and specialized knowledge of relevant processes, systems and tools as they pertain to the professional practice of care coordination and direct nursing.
Professional Practice Educator will contribute to, co-facilitate, and/or take the lead role in one or more of the following:
- New staff orientation
- Assessment Training
- Long Term Care Placement education
- CHRIS (Client Health and Related Information System) Training
- Training and Support for staff returning from extended leaves
- Individualized mentoring/coaching/preceptoring of Care Coordinators for improved competency
- Learning Management System (LMS) and related processes
- Act as the designated educator lead on priority organizational projects /initiatives
NATURE & SCOPE:
- Participates in the development and execution of a comprehensive education program so as to meet predetermined outcomes, consistent with the CCAC mission, vision, corporate strategic plan, and provincial directives and legislation.
- Plans, designs and delivers lesson plans, creates job aids, and utilizes varied instructional media to engage learners and optimize the learning experience.
- Provides classroom and individual learning sessions (i.e., core, remedial, upgrades, refresher), workshops and presentations and ensure the delivery is aligned with identified learning objectives.
- Develops and adapts training programs to support required learning stemming from new local, provincial or governmental initiatives and/or legislative changes.
- Provides support in the application of education into practice for new and existing staff through the use of practice validation exercises, job shadows and individual coaching, as applicable.
- Participates in education related projects, committees and teleconferences locally or with the OACCAC to provide expert advice, feedback and/or recommendations including those relating to material, software upgrades/enhancements , work processes.
- Communicates with educational institutions and other health professionals to facilitate joint education, where applicable
- Promotes principles and practices consistent with a learning organization.
- Other duties as assigned.
- Undergraduate degree in health, education or social services.
- Membership in good standing with the appropriate college as a Registered Nurse, Occupational Therapist, Physiotherapist, Speech Language Pathologist, or Social Worker at the Masters level.
- Certification in or demonstrated knowledge of instructional design including education technology.
- Sound knowledge of adult learning principles.
- Three – five years of working within the Home and Community Care Sector, or related community health experience.
- Current knowledge of relevant processes, systems and tools as they pertain to care coordination.
- Ability to create instructional experiences that make the acquisition of knowledge interesting, productive, and useful.
- Previous experience conducting technical training sessions for adult learners.
- Strong organizational, time management, prioritization, planning and multitasking skills.
- Flexible, adaptable and responsive to change.
- Self-directed with the ability to work independently.
- Highly detailed and data driven.
- Ability to work respectfully, positively and collaboratively within a team environment.
- Strong interpersonal and communication skills - both oral and written.
- Excellent group facilitation skills.
- Expertise in using the RAI assessment tools and successful completion of RAI Competency Evaluation
- Demonstrated proficiency in MS Office applications, (Word, Excel, Outlook, Power Point, etc.)
- Ability to communicate in French or another language an asset.
