Initial Assignment: 450 Speedvale Avenue, Guelph, Regular Day Shift.
Reporting to the Project Manager, this position is responsible for providing strong clinical leadership, advocacy and education. This clinical role demonstrates the following values: person-centred, accountable, collaborative, respectful and innovative.
In collaboration with internal staff and health system partners, the Professional Practice Educator:
- Develops program best practice based on RNAO and leading clinical direct care nursing best practices and case management standards, including the development of program resources and educational materials founded in principles of adult education
- Recommends policy for internal best practice and standardization of clinical and case management standards.
- Develops, delivers and implements a coordinated strategy for the training and education of leading clinical and care coordination best practices to staff and stakeholders
- Partners with clinicians from all settings and geographies within the WWLHIN in order to establish program best practice
- Provides knowledge transfer of leading clinical best practices to improve patient outcomes and patient experience
- Teaches, mentors and coaches members of the multi-disciplinary health care team on leading clinical best practices
- Champions clinical quality improvement initiatives and drives improvement in practice across the WWCCAC and the health care continuum
- Guides, demonstrates and promotes an understanding of the principles and practices of safe, high quality and evidence-based care
- Partners with experts in other jurisdictions to share knowledge and experiences
- Follows all practices, policies and procedures to promote a safe patient and working environment
- Advises clinical work undertaken by Collaborative Groups, Internal Groups and Task Forces by playing a key role in these groups
- Registered Nurse; qualifications in an area of clinical nursing specialty (e.g., advanced education in nursing) or equivalent combination of education and experience
- 3-5 years of demonstrated experience in continuing and adult education
- Leadership or participation in complex projects and initiatives involving multiple health care providers and stakeholders would be an asset
- Proven track record of effective working relationships with internal and external stakeholders and patients as well as the ability to build teams and lead through system-level change
- Ability to resolve conflicts and achieve outcomes
- Excellent time management skills and ability to prioritize multiple, conflicting demands
- Superior organizational skills
- Extensive experience and working knowledge related to care delivery systems across the health care continuum
- Excellent interpersonal, diplomacy, communication and presentation skills
- Strong collaboration skills to participate on projects, initiatives, Collaborative Groups and task forces with colleagues across the WWLHIN area and other health care providers
- Ability to use MS Office applications (e.g., MS Word, Excel, PowerPoint, etc.)
- The ability to travel throughout Waterloo Region and Wellington County is required
