Soins axés sur les résultats

To ensure that people with certain illnesses achieve the best results, we present an outcomes-based approach to care as a new way of providing care in the home and in the community. The goal is to obtain precise results within specified time periods (for example, the wound healed in six weeks).

Through this team-based approach to care aimed at achieving health outcomes, patients with similar needs (for example, those recovering from hip or hip surgery) knee) will experience high-quality, innovative care that is unique to their needs or situation.

Working together as a single team allows you to make more informed decisions

Communication between yourself and your healthcare team is important to improving your healthcare experience. Your team includes your caregivers, a coordinator, a care provider, your family doctor, a nurse practitioner or other health specialist, and yourself.

Results-oriented care includes benefits, including:

  • A coordinated and stable team that understands your specific needs and is prepared to provide you with care.
  • An access point that is full of resources to answer your questions.
  • Improved technology and tools in the electronic and communication fields allowing the constant exchange of information concerning health, while carefully protecting your privacy.
  • Sharing information among the healthcare team reduces the need to repeat your information and medical history.
  • Receive the right care, at the right time - based on best practices and evidence-based.
  • Flexible care - for example, a greater variety of healthcare professionals who can be part of your team; or your care could be offered in different ways, such as with visits with your team, clinic visits, a point of contact and online support service (called telehomecare ) or with support and advice by email.
  • A smooth transition to various healthcare facilities (e.g. from hospital to home)
  • Assessment and report writing in accordance with our performance to improve the quality of care we provide.
