Soins en milieu communautaire

Living at home is a priority for many patients and families. While this is possible in many cases, with appropriate health care in place, other functional community services may prove useful in some cases.

In addition to providing essential support to natural caregivers, these services promote autonomy, quality of life and relaxation for patients.

Here are some examples of community services offered:

  • Meal delivery services and meal programs
  • Housekeeping and family help
  • Transportation services
  • Community meals
  • Friendly visits

Recipients of these services are encouraged to find out about other services available in their community. These services help reduce the stress of daily life for both patients and caregivers.

Ask your care coordinator about these options or call us to refer someone.
