We strive to provide outstanding care to all Ontario Health atHome patients each and every day. Hearing from you, your family members and caregivers helps us strengthen our programs, services and overall patient experiences.

How to Share Your Feedback  

You have the right to share your voice regarding your service(s).

We welcome all kinds of feedback – whether it’s a complaint, concern, compliment or suggestion.

Do you have a complaint or concern?

Your care coordinator is most familiar with your situation and wants to hear what is going well for you and what can be improved. Your care coordinator will work with you, your family or caregiver to address any complaints or concerns you may have. If you are unsure who your care coordinator is, please call us at 310-2222 (no area code required). Most concerns and complaints are successfully resolved at this step.  

If after speaking with your care coordinator and your complaint or concern is not resolved to your satisfaction, you can ask to speak to a patient services manager. If you are unsure how to reach them, please ask your care coordinator or contact our local Patient Relations team.

Do you have a compliment or general feedback to share?

We appreciate hearing what’s working well — your compliments and general feedback helps us recognize staff, improve our services and celebrate success. If someone made a difference in your experience or you’d like to acknowledge great service, we’d love to hear from you.

Here’s how you can share a compliment or feedback:

  • Tell your care coordinator — they’re directly involved in your care and appreciate hearing from you.
  • Reach out to your local Patient Relations team — they’ll ensure your compliment or feedback is shared and acknowledged.
