Home Care

Many people are not aware of the options available to them when their health care needs or circumstances change, and it becomes difficult to manage on their own. Home and Community Care Support Services understands the importance of helping patients, their families and caregivers in every community across Ontario to overcome such challenges and maintain independent living.  

If you require health care or personal support in your home, whether you need assistance getting back on track after a hospital stay, illness or injury, or if you have complex or chronic health conditions, we can get you the care you need, when you need it. Our care coordinators work with you to determine the right care and health supports, develop an individualized plan for care based on your needs and goals of care and help make a meaningful difference to your well-being.   

This can include ensuring you receive necessary services such as:  

  • Care coordination 
  • Nursing 
  • Physiotherapy 
  • Occupational therapy 
  • Nutritional counseling  
  • Speech therapy  
  • Social work  
  • Personal support  
  • Medical supplies and equipment 

Together, we determine what type and level of service(s) are appropriate to meet your needs. Our team of dedicated nurses, occupational therapists, social workers and other care professionals help people heal at home and live in their homes longer. We will also connect you with support services and resources available in your community, in order to assist with your care which could include but is not limited to meal delivery and transportation services.  

Anyone can refer a patient to Home and Community Care Support Services – you, a family member, a caregiver, a friend, your physician or another health care professional.
