Patient Services

At Home and Community Care Support Services, we are committed to the diverse populations we serve across our province and to playing an integral role in creating a healthier community for all. The Patient Services portfolio is fundamental to our mission of assisting our patients to be healthier at home through connected, accessible, patient-centred care. Our dedicated team of health care professionals and support staff work across various settings, including hospitals, clinics, schools and patients’ homes (whether that’s residential, retirement home, congregate setting or long-term care home). They collaborate closely with primary care providers, community agencies, hospitals, long-term care facilities and other health care partners to deliver comprehensive, high-quality patient care to meet the needs of the communities we serve and achieve the best possible health outcomes.

Quality, Safety and Risk

The Quality, Safety and Risk portfolio drives high quality patient care by establishing key quality improvement processes and imperatives. Guidance and oversight focus on patient safety, quality and risk, performance and regulatory compliance including the management of service provider contracts. With the development of a new provincial Quality Framework and Policy, a common approach to quality improvement and performance has been established, driving excellence in care and service delivery.

Corporate Services and Finance

The Corporate Services and Finance portfolio provides direction and oversight for financial planning and reporting, payroll, procurement, business intelligence and data analytics, privacy and health information management, information technology and facilities management. The team defines and drives financial and corporate programs, policies, systems, services and models that support patient services and Home and Community Care Support Service’s overall mandate and mission, vision and values. This portfolio also ensures organizational efficiency, effectiveness, fairness and transparency.

Human Resources

The Human Resources portfolio provides strategic leadership for the development, implementation and evaluation of human capital and organizational development strategies that promote system alignment and enable delivery of patient care through a positive culture of inclusivity, employee engagement and growth.  Within this organization, our people are our greatest asset and we are investing in our people to ensure they have the abilities, support and ambition to do their best work in the communities we serve.

Strategy, Transformation and Engagement

The Strategy, Transformation and Engagement portfolio provides leadership for home care modernization. Through their work, they are collaboratively planning with key partners to enable innovative home and community care delivery models while driving greater consistency and access to care across the province. The portfolio promotes change management to prepare leadership, staff, partners, as well as patients, families and caregivers for the future of home care delivery, while safeguarding the uninterrupted delivery of high-quality care. The team ensures clear, consistent and quality internal and external messaging about the organization’s mandate, programs and services; and develops, enhances and protects the organization’s brand and reputation. In addition, they connect with patients, families, caregivers, community partners and specific populations, including Francophone and Indigenous communities, helping us to learn what matters to them and ensuring we build their insights, expectations and values into how we design and deliver care to the people we serve.
